This session was delivered as a webinar via Teams on Thursday 26 June 2025
Part of our SMD Learning Series.
The session was delivered by Emma Coleman, Board Manager for Nottingham City Safeguarding Adults Board.
The webinar brought together the learning from research into a range of local, regional and national Safeguarding Adults Reviews (SARs) relating to institutional discharge. There are three main areas of learning:
- Policy/pathway/procedure change
- Examples of good practice
- Tips for front line practitioners
The session focuses mainly on the last two areas and uses the SARs as case examples to demonstrate the negative impact of poor, uncoordinated discharge but also the positive impact when things are done well. It is hoped that the learning from these case examples will provide you with some insights that you can bring into your own practice and share with your teams.
The webinar also considers some of the relevant learning from the 2nd National SAR Analysis undertaken by Professor Michael Preston-Shoot - Second national analysis of Safeguarding Adult Reviews: April 2019 - March 2023.
Although not all the cases feature individuals experiencing Severe and Multiple Disadvantage, the learning will be very much applicable to this group of people and will hopefully help you to support them.
