Locally, all Domestic Homicide Reviews have identified the need for better recording of information on systems and case notes as a learning point, including the linking of relationships and naming of people such as partners and other family members in case notes / case management systems. The intention of this guidance is to support agencies with record keeping. Effective record keeping assists agencies and staff to understand the circumstances surrounding the person and evidence what has been done, identify any risks as well as good practice.
