Advocacy, information and advice charity POhWER is looking to raise awareness of its new pilot Mental Health Link Community Service for Nottingham City and Nottinghamshire.
This service is a wraparound mental health service which includes social prescribing and advocacy. It provides vital community support for patients recently discharged from their section under the Mental Health Act in the community.
Through this service, Mental Health Community Link Workers are available to help service users with any of the following:
- Supported signposting to community support services in Nottingham City only
- Help for service users with talking to professionals and putting their views across about their care and support needs
- To discuss any changing support needs not already identified in a service user's hospital discharge plan
- To support service users with raising any concerns about their care and treatment with their mental health team
