Nottingham & Nottinghamshire Integrated Care System (ICS) wants to bring together patients, carers, the voluntary sector, and its workforce from both health and social care to look at working together and designing services for the future that will help patients to recover better after being in hospital.
This ICS consultation has been launched following the release of new national guidance that outlines how organisations can work together differently to better support the recovery of discharged hospital patients.
Who should be involved?
Patients and carers who have recent experience of discharge from hospital to their home and have received help and support from health and social care at home to help with their recovery.
Voluntary sector and health and social care staff who deliver services to support recovery/reablement/rehabilitation such as physiotherapists and occupational therapists are also invited to attend.
Share your views at ICS engagement event
An engagement event has been arranged to take place on Wednesday 24 February 2021, from 2pm-4pm.
The online event will help set the scene around the national context including evidence from NICE. The event will be interactive with polls, use of the chat function and group work included. It will explore what is currently working well and how the ICS can build on this, identifying the challenges and top priorities to start to take this work forward.
This event is just the start - the ICS hopes that people will continue to be engaged and that a project team will be set up to progress with the work. The project team will likely meet on a fortnightly or monthly basis, for a minimum of six months.

All engagement is taking place virtually during the Coronavirus lockdown to make sure staff, patients and carers remain safe. With this in mind, this engagement event will take place online using Zoom. Information to help you use Zoom can be provided upon request.
Please note places are limited and will be allocated to ensure the ICS has representation from across Nottingham City and Nottinghamshire County. Depending on the level of interest an additional event for patients and carers may be arranged.
