Additional support for mental health discharge patients

Advocacy and support charity POhWER is providing a new mental health service for the Nottinghamshire Healthcare NHS Foundation Trust.

The organisation's role is that of a Community Mental Health Link Worker and they aim to deliver a wraparound service in the form of a social prescribing model that will add vital support for patients on discharge and transition between inpatient mental health settings and community or care home settings.

The role of their link workers is to understand what matters to that person and to work with them to develop an advocacy support plan. This support plan will identify all of the person’s individual support needs and provide supported signposting and referrals to relevant local and national services, thus enabling them to navigate complex health and social care systems by providing the right information. 

The primary goal is to offer additional support for those transitioning back into the community with the aim of reducing the likelihood of re-admittance. 

This service is currently only available for people in Nottingham City and Nottinghamshire who are or have been discharged (very recently) from mental health hospitals back into the community in the Nottingham area.
